How to Get Started with
Our Wound Care and telehealth services
Beginning care with Midwest Wellness and Wound Care is simple. We make the process easy so patients and families can focus on healing.
1
Contact Us
Call our office or send us an email. Our team will answer your questions and help determine whether in-home wound care services (available in multiple states) or telehealth primary care and psychiatry services (available in Arizona) are appropriate for your needs.
2
Schedule your visit
Once your care needs are identified, we schedule your visit at a time that works for you. In many cases, same-week appointments are available for in-home wound care or telehealth medical and psychiatric services.
3
First evaluation
For in-home wound care, a clinician visits your home or care facility to complete a comprehensive assessment. For telehealth services, a licensed provider conducts a secure virtual evaluation, reviewing your medical history, medications, and current concerns.
4
Personalized care plan
We develop a care plan tailored to your individual needs. This may include advanced wound treatment, telehealth follow-up, primary care management, psychiatric care, and education for patients and caregivers.
5
Ongoing support
We provide ongoing care through regular visits, telehealth follow-ups, and clear communication. Our team remains available to support you, answer questions, and adjust your care plan as needed throughout your treatment journey.
