How We Care
We provide care through in-home wound care visits and secure telehealth appointments, based on patient needs and eligibility. In-home wound care begins with a thorough assessment of the wound, medical history, and care environment, followed by a clear, individualized care plan focused on healing and prevention. Telehealth visits include a comprehensive virtual evaluation of medical or mental health needs, medication review, and development of an appropriate treatment or follow-up plan. Throughout care, our clinicians communicate clearly and coordinate with physicians, facility staff, patients, and caregivers to ensure seamless, continuous care.
